Medical (military medical) commission

The Medical (Military Medical) Commission was established to conduct a military medical examination to determine the suitability of candidates for service in the National Police and the National Guard, candidates for admission to higher educational institutions of the Ministry of Internal Affairs, suitability of police and servicemen for their transfer, dismissal.

LIST OF MEDICAL DOCUMENTS FOR PERSONS ENTERING THE SERVICE OF THE NATIONAL POLICE OF UKRAINE

DIRECTION FROM THE HUMAN RESOURCES DEPARTMENT FOR MEDICAL EXAMINATION BY FORM (APPENDIX 4).

1. MILITARY TICKET OR ATTRIBUTE CERTIFICATE FOR MEN, PASSPORT FOR WOMEN.
2. OUTPATIENT CARD FROM THE RESIDENCE.
3. DETAILED EXCERPT FROM THE MEDICAL CARD OF THE TREATMENT AND PREVENTION INSTITUTION BY PLACE OF RESIDENCE, WORK, SERVICE OR TRAINING FOR THE LAST 5 YEARS
4. CERTIFICATE OR CERTIFICATE OF PSYCHIATRIST AND DRUG ADDICT (at the place of registration).
5. INFORMATION ON MANDATORY PREVENTIVE VACCINATIONS (2 copies on form 063 / o).
6. ZAG. ANALYSIS OF BLOOD, URINE, AN. BLOOD ON GLUCOSE, AN. BLOOD ON RV, HEPATITIS B, C; OX.
7. X-RAY (FLUOROGRAPHIC) EXAMINATION OF THE CHEST ORGANS, ECG.

Note: Former servicemen must have a medical certificate, a copy of the conclusion of the M (VL) K, a copy of the extract from the release order.

Candidates for police service must provide documented results of these examinations, conducted within one month before the medical examination in M ​​(VL) K

5. DETAILED EXCERPT FROM THE MEDICAL CARD OF THE MEDICAL AND PREVENTIVE INSTITUTION BY PLACE OF RESIDENCE, WORK, SERVICE OR STUDY FOR THE LAST 5 YEARS. (The statement must be certified by the doctor's signature and the seal of the medical institution).
6. REFERENCES (CERTIFICATES) OF PSYCHIATRIST AND DRUG ADDICT (at the place of registration).
7. INFORMATION ON MANDATORY PREVENTIVE VACCINATIONS (2 copies on form 063 / o).
8. REFERENCE FROM ANTI-TUBERCULOSIS DISPENSARY.
9. GENERAL AN. BLOOD, URINE, AN. BLOOD ON GLUCOSE, AN. BLOOD ON RV, HEPATITIS B, C; OX; BLOOD GROUP, Rhesus factor, BIOCHEMICAL AN BLOOD (general bilir., ALT, AsT).
10. X-RAY (FLUOROGRAPHIC) EXAMINATION OF THE CHEST ORGANS, ECG.

Note: Former servicemen must have a medical certificate, a copy of the conclusion of the M (VL) K, a copy of the extract from the order of dismissal. Candidates for military service under the contract must provide documented results of these examinations, conducted within one month before the medical examination in M ​​(VL) K.

LIST OF MEDICAL DOCUMENTS FOR PERSONS ENTERING HIGHER EDUCATIONAL INSTITUTIONS OF THE NATIONAL POLICE OF UKRAINE

• Referral from the personnel department for a medical examination according to the form / Annex 4 /.
• Registration certificate or military ticket for men, passport for women, 1 photo 3x4.
• Outpatient card. Lyceum students must present a medical description.
• Detailed extract from the medical card of the treatment and prevention institution at the place of residence, work, service or study for the last 5 years. (The statement must be certified by the doctor's signature and the seal of the medical institution).
• Reference (certificate) of a psychiatrist and narcologist / from the place of registration /.
• Certificate of mandatory preventive vaccinations (form 063 / o) in 2 copies.
• General analysis of blood, urine, blood glucose, blood test for RV, hepatitis B, C, HIV.
• X-ray / fluorographic / examination of the chest, ECG.
• Candidates for training must provide documented results of these examinations, conducted within one month before the medical examination in M ​​(VL) K
